Case Manager/Recovery Advocate

•Refers clients to community health, HIV and TB clinics as necessary.
•Transports clients when necessary.
•Maintains appropriate and timely documentation on all parolees as outlined in the program Procedures Manual and as required by contract.
•Maintains contact with assigned treatment providers to initiate risk management strategies.
•Promotes effective relationships with staff by demonstrating a coordinated team approach to treatment.
•Prepares critical care issues for discussion during bi-weekly supervision.
•Establishes weekly case presentations for admission review. This process includes all admissions in the caseload.
•Provides input on an ongoing basis to develop and improve program servicing and delivery.
•Meets and maintains security and other standards necessary for admission to the program. Complies with all CDCR requirements including security and safety.
•Complies with all MHS and program rules, policies and procedures including HIPAA standards.
EDUCATION & WORK EXPERIENCE REQUIREMENTS:
•A high school diploma (or equivalent) and at least two years of professional experience demonstrating a history of service provision in the area of substance abuse treatment and recovery to the criminal justice population is required.
•Within two (2) years of hire into the program, shall be certified, in substance abuse services within a criminal justice setting.
