Program Description
Families Forward is a Wraparound Program in which mental health professionals and paraprofessionals partner with youth and families to find creative solutions to the family's unique challenges. Wraparound is a family-driven process that facilitates team building and decision making using the strengths and resources of the family and community. Together, the team creates goals to promote stabilization in the home and self-sufficiency through community and family partnership.
Position Description
Youth Support Partner –
- Develop a supportive, nurturing relationship with parent/caregivers and families.
- Provide support to the youth and models healthy interactions.
- Serve as part of the multidisciplinary team, providing representation of, insight and support to youth.
- Assist youth in identifying their educational and vocational needs.
- Provide advocacy for child, parents, and families.
- Support the healthy development of children and families.
- Complies with all documentation and data requirements; participate in quality assurance process.
- Interface with and participate in wraparound treatment teams with public systems, providers, and families.
- Maintain flexible schedule according to client needs. Serve as part of the back up team during crisis.
- Attend appropriate community meetings.
Education and Experience Requirements
- HS Diploma or GED
- Life experience with public or social services system of care.
- Not currently involved in Probation, AB2726, or Child Welfare Systems.
- Bilingual/Bicultural candidates are encouraged to apply.
